Returning Student Application

To return to Western Seminary after not enrolling for one academic year or longer.

To formally change from one master’s level degree program to another. If you are non-degree, please do not submit this form. Contact the Admissions Office for the appropriate form.
Students enrolled in a Perspectives on World Ministry course at a church may be eligible to receive graduate credit through Western Seminary by submitting this form.

Mentored Field Ministry
Tuition Waiver Form

To request a tuition waiver for those who have served as a pastoral mentor in the Mentored Field Ministry program.
To change from M.A. in Ministry and Leadership, Pastoral Care to Women Specialization, to the Youth Ministry specialization. If you are non-degree, please do not submit this form. Contact the Admissions Office for the appropriate form.
To change from MDiv to MDiv/MAC dual degree, etc.
To test out of a course. Must be done prior to or concurrent with your first semester of enrollment.
To request that graduate-level coursework be counted towards your degree program.

To change your name. Must be a legal name change.
To request a transcript for non-advising purposes.
To request a mailbox in the student center.
To register for a class via the Center for Lifelong Learning.
